ABSTRACT

The COVID-19 pandemic requires a quick and accurate diagnosis and assessment of the condition of patients, correctly chosen by the tactics of treatment and prediction of the course of the disease, especially in older patients. The most promising direction is the study of the potential of inflammatory factors among people over 60 years of age. This paper analyzed serum level characteristics of analytes such as C-reactive protein, procalcitonin, IL-6, troponin, ferritin and brain natriuretic peptide. The findings demonstrate the prognostic value of IL-6 and procalcitonin in both middle-aged and older-aged individuals. For people of senile age, the diagnostic informativity of both IL-6 and Pct reached an acceptable level. C-reactive protein is statistically significantly higher in fatal patients.

ABSTRACT

Laboratory monitoring during the SARS-CoV-2 pandemic is essential in providing emergency care to patients with COVID-19. Objective. To determine the critical values of laboratory tests in patients with COVID-19 pneumonia for optimizing the treatment. Material and methods. Case histories of 6151 patients were retrospectively analyzed at the St.-Petersburg City Multi-field Hospital No. 2 redesigned for providing medical care to COVID-19 patients between March 30, 2020 and July 26, 2020. All patients were retrospectively divided into 3 groups: group 1 — patients without mechanical ventilation (n=5646 (92%));group 2 — invasive mechanical ventilation without lethal outcome (n=69 (1%));group 3 — invasive mechanical ventilation with lethal outcome (n=436 (7%)). Results. Considering literature data, we distinguished the following perspective tests: ferritin, leukocyte count, absolute neutrophil count, absolute lymphocyte count, platelet count, C-reactive protein, alanine aminotransferase, aspartate aminotransferase, total bilirubin, troponin I, creatinine, and urea. Medians and 25— 75% percentiles were calculated for all laboratory tests in 3 groups. We compared the results between all groups and assessed their reference intervals for the final choice of critical values. Conclusion. The authors propose to use their results as reference critical values of laboratory tests for assessing the condition of patients with COVID-19 pneumonia. © 2021, Media Sphera Publishing Group. ABSTRACT

The paper presents the results of a standard and complex treatment method using the peptide drug thymus thymalin in patients with COVID-19. One of the mechanisms of the immunomodulatory effect of thymalin is considered to be the ability of this peptide drug to influence the differentiation of human hematopoietic stem cells (HSCs). It was found that, as a result of standard treatment, patients in the control group showed a decrease in the concentration of the pro-inflammatory cytokine IL-6, C-reactive protein, D-dimer. The addition of thymalin to standard therapy accelerated the decline in both these indicators and the indicators of the T cell system. This has helped reduce the risk of blood clots in COVID-19 patients. The revealed properties of the thymus peptide preparation are the rationale for its inclusion in the complex treatment of coronavirus infection. ABSTRACT

Relevance. In order to prevent the spread of infection caused by the SARS-CoV-2 virus, in the spring of 2020, a number of hospitals were re-profiled in St. Petersburg toreceive patients with pulmonological pathology, including infectious etiology. In the period from March 30 to April 30, 2194 patients were admitted to St. Petersburg City Hospital №2, including 1211 (57%) with pneumonia of various etiologies, 508 (24%) patients with COVID-19 and 475 (22%) patients with other acute upper respiratory tract infections. The aim of this study was to evaluate the structure of the results of prescribed studies to optimize the laboratory service and assess the laboratory and examined patients profiles. Results structure assessment of a general blood tests showed the predominance of relative lymphocytopenia- in 51.9%;hemodilution in 57.9%;the average platelet volume was below the reference limits of 59.6%. Leukocytosis was observed in 46.5% and neutrophilia in 53.7%. Absolute monocytosiswas found in 45%. Tests analysis of the biochemical analytes concentrations showed elevated levels of: C-reactive protein - in 74.8%;glucose - in 73.9%;troponin I - in 75.6%;lactate dehydrogenase - in 63.4%;procalcitonin -in 55%;gamma-glutamyltranspeptidases - in 52%. Conclusion. To optimize the laboratory service of the hospital, the quantity of goods was increased by no more than 10% in government contracts concluded with suppliers of laboratory reagents, which made it possible to purchase additional supplies to ensure uninterrupted laboratory examination of patients in the infectious hospital in the conditions of the new coronavirus infection COVID-19. Актуальность. В целях профилактики распространения инфекции, вызванной вирусом SARS-CoV-2, в Санкт-Петербурге весной 2020 г. произошло перепрофилирование ряда стационаров для приёма пациентов с пульмонологической патологией, в том числе инфекционной этиологии. В связи с этим становится актуальной оптимизация лабораторного обследования пациентов в условиях инфекционного стационара при пандемии коронавирусной инфекции COVID-19. Цель исследования. Изучить структуру результатов назначаемых исследований для оптимизации лабораторной службы и оценки лабораторного профиля обследуемого контингента пациентов. Результаты. Оценка структуры результатов общего анализа крови показала преобладание относительной лимфоцитопении - в 51,9% исследований;гемодилюцию в 57,9% анализов;средний объем тромбоцитов был ниже референсных пределов в 59,6% результатов у пациентов инфекционного стационара. Наблюдался лейкоцитоз в 46,5% и нейтрофилез в 53,7% исследований. Абсолютный моноцитоз отмечен в 45% исследований. Анализ результатов исследования концентраций биохимических аналитов показал повышенные уровни: С-реактивного белка - в 74,8%;глюкозы - в 73,9%;тропонина I - в 75,6%;лактатдегидрогеназы - в 63,4%;прокальцитонина - в 55%;гзммз-глютамилтранспептидазы - в 52% от общего количества проведенных исследований уровня соответствующего показателя. Выводы. Выявлена увеличенная потребность клинических подразделений больницы в определенных лабораторных тестах. Для оптимизации лабораторной службы стационара увеличено количество расходных материалов, не более чем на 10%, в заключенных с поставщиками лабораторных реагентов государственных контрактах, что позволило приобрести дополнительное количество расходных материалов для обеспечения бесперебойного лабораторного обследования пациентов инфекционного стационара в условиях новой коронавирусной инфекции COVID-19.
